One doesn't see the following waves very often:

 

 

In the above page, the occipital rhythmic delta waves are seen on the left, but in the following examples these are seen bilaterally;

 

 

 

 

 

 

Here is some literature on the subject of OIRDA. The frequency of these waves may distinguish children with focal (2-3Hz OIRDA) from generalized epilepsies (3-4Hz OIRDA). In children with absence seizures, they may be unilateral or shift from one side to the other and they may straddle theta and delta frequencies (3-4Hz).

In some instances, the answer is found elsewhere on the EEG. These were the only 2 inter-ictal epileptiform discharges seen during a 24h EEG and hence if you see OIRDA only, which is not infrequently the case, especially on routine EEGs, you should know that epilepsy is likely and that more often than not it will be generalized in nature.
